How to download Medical, Dental and Vision ID cards

You can download or print ID cards from Aetna, Kaiser and Delta Dental’s websites. VSP doesn’t provide ID cards, so your provider will locate your record using your SSN.

Aetna

• Click Login and under Don’t Have an Account? click the register button, then select sign up using my SSN.
• Enter the required fields to grant access to your Aetna account.
• Once logged in, under Plan Overview select ID Cards. From here you can download or print your Aetna card as a PDF or request a physical card.

Delta Dental

• Click Sign in or Register in the right-hand corner.
• Select “create an account” and complete the prompts.
• After creating a profile, your dental ID card will be on your dashboard to download or print.
• Download/print from websites. Dental providers can also find members using SSN.

 Kaiser

• Click “Register” in the right-hand corner and follow the prompts.
• Click on “Coverage & Costs” and then scroll down to “ID card help” under helpful resources.
• Select each member’s checkbox to print the card(s).
