How to download 1099-SA form

IRS form 1099-SA reports distributions from health savings accounts (HSAs) and medical savings accounts (MSAs).

You can download the 1099-SA from your HSA dashboard using the instructions below:

• Go to MA Connect > MA Benefits > Health Savings Account (HSA)
• Click the 3 horizontal bars in the top left corner and expand the "My Account" drop down list
• Select "Benefit Account Summary". Scroll down to the "Health Savings Account" section
• Click on "Tax Forms" located just above your account balance and next to Account Resources
• A new window will open with your tax forms. From here you can view, download, or print the form.

Remember you will only have 1099-SA form if you took distributions in previous year.
